Isn't it a bit hypocritical that we say 'don't judge a book by the cover', yet we only apply this to if you've made the judgement that the book is a bad one? Why, when we look at a person and think, 'oh, she looks like a nice girl!' does the mind never caution, 'careful, don't judge a book by it's cover!'? And isn't that making an assumption in itself that the person is bad? Or if used traditionally in the bad sense of the saying, that the person is good? The whole, 'don't judge a book by the cover' thing gets incredibly confusing. A person has no control over the thought process. It's what they say, what they do, that makes the book good or bad. Having a thought, good or bad when you first see a person, does not mean you are judging a book by it's cover. It simply means you're one of those lucky people who still has a thought process. Has anybody never walked into a room and gone in their head, 'Woah, he/she looks like a ______.' Do we not, as the human race, all have this in common? Though perhaps hypocritical isn't the right word. I think that a person can judge all they want, it's what you think of yourself that matters. A person can feel free to call me a vapid, poisonous idiot, but I honestly don't think I'd give two shakes about it.
I just had this weird thought... I don't know...
Another thing: What is a book cover for, Protection of the book, yes... But you can also gleam a great deal of information from the books cover, as the cover is meant to be a thing to make you pick up the book. Reading the back of the book is typically done on the cover as well... I'm thinking too much about this....

Personally I've always applied it both ways.  There are always exceptions to ever rule, and I never assumed good guys always wear white t-shirts, if you catch my drift.  Sometimes bad guys wear white t-shirts too.
I agree you're thinking into the phrase a little too much.
Keep making observations and questioning, though.  It's good.
